The purpose of this assignment is to collaborate as a team to create an action plan for
implementing a public health intervention within a selected community/culture.
Throughout Topics 1-4 of the course, you will participate in a series of Collaborative
Learning Community (CLC) assignments. These assignments will build upon one another
and be used as the basis for the Reflective Journal assignments throughout the course.
As a public health professional, it is important to align public health programs with an
organizational mission, vision, and objective to ensure sustainability and growth within the
target community. Using the feedback you received from your instructor about the
intervention you submitted in the Topic 2 assignment, collaborate as a team to discuss your
vision and the leadership and management approach you think will be most effective for

Continuous quality improvement plan
Quality improvement tools are critical in the identification of a project’s prospects while
aligning them to the intended objectives. The quality improvement tools and techniques provide
an opportunity for realizing success in the community intervention strategic plan. First, the
continuous quality improvement plans promote the accountability community initiatives of
collaboration through enhancing a bottom-up approach in determining and addressing the issues

within the community (Antonacci et al., 2018). The improvement plans focus improves the
monitoring of various community matters thereby developing shared community goals in
accomplishing their objectives. Thus, the improvement techniques would enhance a proactive
and pragmatic approach in maintaining the community and stakeholder focus in their objectives.
Second, the improvement plans are essential in promoting the tracking of progress and change
enhanced by a project. The community members and stakeholders would utilize the quality
improvement techniques to enhance the effective implementation of the objectives and
sustaining positive progress.
Additionally, the improvement plans enhance the testing and refining of different actions
at various stages allowing for incremental modifications (Ovretveit et al., 2017). The
improvement plan can provide a basis for undertaking various tests on small scale in evaluating
their impact. The community can utilize various techniques in enhancing the situational
awareness of community stakeholders relating to the project’s needs. Moreover, the quality
techniques would enhance the expansion of the collaborative initiative in addressing the health
disparities to sustainable levels. Besides, the community intervention plan through the
implementation of the quality initiative would promote the commitment to the continuous
evaluation while adopting best practices in communicating results. Through an insightful review
and evaluation of the strategic plan, the improvement techniques can assist in the continuous
collection and analysis of data relevant to achieving relevant success.
